Sage Community Partnership

Sage Community Partnership is a Boulder County Colorado, based and focused,
non-profit 501-(c)-(3) corporation focused on supporting at-risk youth; children, adolescents and young adults with histories of abuse, neglect, mental health and substance abuse issues, juvenile justice involvement or homelessness. Through our varied service programs, we are committed to providing each of our clients the opportunity and support necessary to lead healthy, independent and sustainable lifestyles. As an umbrella non-profit, Sage CP operates the following programs: Polaris and Vega Transitional Living Programs (which includes Polaris House), Youth Grants Program, Leadership Recognition Program, and the Transition Age At-Risk Community Youth Task Force.
